Complete the  Healthy Communities Visioning Survey. It is now available in Spanish (español), Chinese (中國), Tagalog, and Tongan (lea fakatonga)!

We have extended the survey deadline to Wednesday, April 29. We want to make sure we have broad participation. As we shared previously, from five workshops across the county, participants identified 10 key components for creating healthy, equitable communities. We want to make sure we didn’t miss a key component and know how you prioritize these components!

We hope you and your networks will take five minutes to fill out this short survey by April 29.
